Contract Guideline:

  • I can have one treatment per month at any time during normal business hours (Monday-Saturday).

  • If I miss a month, I can do 2 the next month (for the same treatment).

  • I cannot miss more than 1 month.  The payment for the treatment not received will be billed.

  • I can only use this membership for the service I bought the membership for.  I cannot exchange for any other service.

  • Membership is non-transferable.

Contract Cancellation:

  • I can cancel contract under the following conditions (without penalty): 

  • I move more than 25 miles away.  A letter with valid proof must be shown to a manager (utility bill perferred).

  • Permanent disability with written note from your doctor.

  • I can only use this membership for the service I bought the membership for.  I cannot exchange for any other service.

  • Membership is non-transferable.
